Where two bones meet, their ends are capped with cartilage and the joint is bathed in a fluid that lubricates and nourishes it, rather like oil in an engine.

In a healthy joint, cartilage is constantly maintained, worn a little and repaired a little, in balance. In osteoarthritis, that balance tips: the cartilage breaks down faster than it is rebuilt. It thins and roughens, the lubrication reduces, and the bone beneath responds by forming small growths at the joint's edges. The result is pain, stiffness, and a joint that moves less freely than it used to.

The key point, and a genuinely encouraging one, is that cartilage is living tissue. Osteoarthritis is a process that can be slowed, eased, and worked with, especially when caught before the joint is severely damaged.
