What is Duodenal Ulcer?

 

Just beyond the stomach lies the duodenum, the first stretch of the small intestine, where acidic stomach contents arrive to continue digestion. A duodenal ulcer forms when this lining is eroded when acid overwhelms the duodenum's protective defences and damages the tissue, creating an open sore.

Duodenal ulcers are the most common form of peptic ulcer (the broader group of ulcers in the stomach and duodenum). Signs and Symptoms: the Distinctive Pattern

The timing of the pain is what most sets duodenal ulcers apart:

  • Pain that is relieved by eating — food and the duodenum's response temporarily buffer the acid, so eating often eases the pain (in contrast to gastric ulcers, where eating may worsen it)
  • Pain that returns a few hours after meals, when the stomach empties and acid reaches the ulcer again — sometimes called "hunger pain"
  • Night-time pain — classically waking a person in the early hours, when acid is high and the stomach empty
  • Burning or gnawing pain in the upper abdomen
  • Bloating, nausea, or a feeling of hunger
  • Sometimes relief from antacids or milk

The Warning Signs to Never Ignore

Seek urgent medical care if you have:

  • Black, tarry stools, or blood in the stool
  • Vomiting blood, or material that looks like coffee grounds
  • Sudden, severe, or spreading abdominal pain (possible perforation)
  • Feeling faint, dizzy, or breathless (possible internal bleeding)

Causes

Like all peptic ulcers, duodenal ulcers come predominantly from two causes — and the first is even more dominant here:

  • H. pylori infection — this common stomach bacterium is behind the great majority of duodenal ulcers, even more so than gastric ulcers. It needs specific antibiotic treatment to clear; without that, the ulcer tends to recur.
  • NSAIDs — regular anti-inflammatory painkillers (ibuprofen, aspirin, and similar) that weaken the lining's defences.

Smoking, alcohol, stress, and diet can aggravate symptoms and hinder healing, but they're usually contributors rather than the root cause. Diagnosis is medical, typically including a test for H. pylori and, where needed, an endoscopy. (Helpfully, duodenal ulcers are very rarely cancerous.)

The Conventional Medical Approach

Modern treatment is highly effective and usually curative, and it is the essential foundation. It works by clearing the cause and reducing the acid so the ulcer can heal:

  • H. pylori eradication — where the bacterium is present (which is most of the time), a specific course of antibiotics with acid suppression clears the infection, healing the ulcer and, crucially, stopping it from returning.
  • Acid-reducing medication — proton pump inhibitors (PPIs) powerfully reduce acid and allow rapid healing.
  • Reviewing NSAIDs — where these contribute, stopping or changing them (with medical guidance) is essential.
  • Lifestyle support — reducing smoking, alcohol, and aggravating foods to aid healing.

Because clearing an H. pylori infection addresses the actual cause, it doesn't just heal the current ulcer — it usually prevents future ones, which is why this treatment is so effective. How Ayurveda Understands Duodenal Ulcers

A soothing, cooling diet — genuinely central, and suited to the pattern. Because duodenal pain eases with food, regular, unhurried meals without long gaps are especially helpful. Favour bland, cooling, easily digested foods — milk, ghee in moderation, soft rice, rice gruel, barley, coconut water, and soothing herbal drinks (coriander, cumin, fennel); and reduce the aggravating foods — spicy, sour, salty, fried, alcohol, smoking, and excess tea and coffee — and late-night meals.

Stress and lifestyle. Because stress aggravates acidity, calming practices, meditation, pranayama, good sleep, and a settled routine genuinely support healing.

A reassuring note, and a caution. Duodenal ulcers are very rarely cancerous, which is genuinely reassuring. But persistent digestive symptoms should still be medically assessed rather than assumed, and alarm features — unexplained weight loss, difficulty swallowing, persistent vomiting, or new symptoms in an older adult — always warrant proper evaluation.
